Family Health Center is looking for a compassionate individual like you to join
our Lending Hands for Life Team! The LHL Medical Case Manager provides medical
case management services to HIV+ clients at Erie Family Health Center (EFHC).
Responsibilities:
-Works with providers, LHL team members, and other staff at the Health Center to coordinate the delivery of services to clients
-Assist with access to care, and facilitate the appropriate use of services
-Complete outreach, client advocacy, assessment, care planning, and facilitating adherence to medical care plans
-Document in the electronic medical record and state-database for all communication related to medical care.
Requirements:
-Bachelor degree required (MSW or related field preferred) or the level of knowledge and experience equivalent to that ordinarily acquired through completion of a Bachelor’s degree in social work or a related field is an acceptable option.
-Two years of progressive experience in a health care delivery organization or health care association.
-Cultural competency to work in a low income, minority environment.
-Familiarity with issues affecting individuals living with HIV.
-Bilingual (English/Spanish) required (May be required to translate for non-English/Spanish only speaking patients and/or guests.)
