Clinical Integration Project Intern

JOB SUMMARY
The Clinical Integration Intern provides general program assistance, planning, implementation and data analysis support to multiple projects led by the MHN Clinical Management Team, related to Practice Transformation including Care Coordination, Disease Management, Patient Centered Medical Home and Quality.

Essential Functions

  • Assists in planning and development of MHN ACO reporting to support the Clinical Committee and supporting Subcommittees in their analysis of the care management program effectiveness including but not limited to ED utilization and readmission trends, transitions of care programming evaluation, and care management quality
  • Generates and maintains monthly Clinical Integration Quality dashboards along with other quality care management performance monitoring requirements
  • Compiles research and literature reviews to keep the team and the ACO up to date on new strategies for improving performance and best practices
  • Assists with coordination of the MHN Clinical Committee, Care Management Lead Meetings, and all supporting Subcommittees. This includes but is not limited to creating and gathering agenda items and supporting documents, taking meeting minutes, and following up on open task items
  • Organizes and leads or assists with special projects delegated from the Clinical Management Team, summarizing results and creating presentations; examples include the development and inplementation of transitions of care programming or NCQA care management reviews 
  • Supports Clinical Team on other projects as needed

Competencies

  • Excellent verbal and written communication skills, including public speaking skills.
  • Strong computer skills, particularly with the entire Microsoft Office suite, including Word, Excel, Access, PowerPoint , and Outlook.
  • Prefer experience using Base SAS, SAS/Macros, and SQL
  • Excellent client/customer service orientation.
  • Ability to deal effectively with a variety of individuals and professionals, including patient s, clinic staff, physicians, and senior leadership.
  • Ability to work successfully in a team environment.
  • Ability to multi-task, prioritize, and work under deadlines.
  • Proactive, self-directed, and able to approach complex tasks independently.
  • Knowledge and experience in the area of Patient Centered Medical Homes.

Required Education
Bachelor's Degree with a health care related major or minor preferred, but healthcare experience may be substituted.
Work Environment
This position operates in a professional office environment, and physical presence and regular attendance are essential functions of the job. This role routinely uses standard office equipment.

  • The employee in this position must be able to move and carry presentation materials and equipment weighing up to 40 pounds in connection with business meetings and travel.

Position Type/Expected Hours of Work
This is a part-time position.

Travel
Travel as needed for off-site meetings; 100% of travel will be local travel.
