This position will provide expertise and leadership in developing a portfolio of programs to advance the health of patients, families, and communities. The primary duties for this position include designing evidence-based population health programs, writing grants to secure funding, evaluating programs, and communicating key findings. Population health programs will focus on important primary care and public health concerns, such as improving health equity, reducing the use of tobacco, alcohol, and other illicit drugs, addressing epidemics related to obesity and chronic disease, vaccine hesitancy, and other issues vital to primary care and public health. There is considerable latitude with program design, and we are looking for innovative thinkers. Programs can range from physician/patient education to policy and systems change.

REQUIREMENTS:
Bachelor’s degree required, and graduate degree (MPH, MHA, MPA, DrPH, PhD, etc.) highly preferred in the area of public health, health care administration, or other related disciplines. Applicants must have 4-6 years of relevant experience. Ability to work collaboratively across multiple functional groups in a matrixed environment. Team player who can work well with internal teams.
