POSITION SUMMARY

The West Cook YMCA Chronic Disease Program Facilitator will facilitate our discussion-focused, evidence-based chronic disease prevention programs including: Healthy Weight and Your Child, a childhood obesity intervention program, YMCA’s Diabetes Prevention Program, Take Charge of Your Diabetes, a diabetes self-management program, and Blood Pressure Self-Monitoring Program. They will also provide support to our Healthy Aging programming, as needed.

ESSENTIAL FUNCTIONS

  • Adhere strictly to curriculum when delivering evidence-based programming. Maintain accurate attendance and program data records.
  • Adhere strictly to curriculum when delivering evidence-based programming.
  • Create a safe, judgment-free and encouraging environment that is friendly and welcoming to all. Encourage group participation and interaction through use of open-ended questions and other facilitation skills.
  • Build authentic relationships with class participants. Help participants connect with each other and the YMCA. Display empathy, support and encourage goal setting, and follow up with participants who miss class. Maintain participant confidentiality.
  • Prepare before each class. Arrive for classes 10-15 minutes early and dressed appropriately. Be accessible to participants both before and after classes to answer questions or address concerns they may have.
  • Promote all chronic disease programs and answer questions from potential participants when needed.
  • Be diligent in maintaining communication through West Cook YMCA email.
  • Attend and contribute to all mandatory staff meetings and trainings.
  • Substitute for other instructors to encourage overall quality programming and staff support. Maintain proper communication with supervisor when seeking/offering to substitute any class. If for any reason a substitute is needed, it is the instructor’s responsibility to find a substitute.
  • Follow all YMCA policies, rules, regulations and procedures, including emergency and safety procedures. Respond to all injuries and assist in immediate first aid and proper emergency procedures. Complete incident and accident reports as needed.
  • Perform Additional duties as assigned by the Healthy Living Director or Chronic Disease Program Coordinator.

QUALIFICATIONS
  • Degree in public health, health education, social work, or related field.
  • Strong facilitation skills and comfort with public speaking. Knowledge of motivational interviewing techniques highly desired.
  • Strong ability to build community within a group and have empathy for individuals from all walks of life.
  • Certification in CPR/AED and First Aid through the American Heart Association or the American Red Cross.
  • Must have a flexible schedule including ability to work early mornings, in the evenings and on weekends.
  • Must be willing to travel locally to community locations to administer programming.
  • Must be willing to travel, potentially to other states, to receive program trainings.
  • Bilingual in English and Spanish is preferred but not required.
WORKING CONDITIONS
  • Ability to walk, stand and sit (including on the floor) for long periods of time.
  • Exposure to communicable diseases and bodily fluids.
  • Ability to stand or sit while maintaining alertness for several hours at a time.
  • Position may require bending, leaning, kneeling and walking.
  • Must be able to lift and carry supplies weighing up to 50 lbs.
  • Ability to speak concisely and effectively communicate.
  • Visual and auditory ability to respond to critical situations and physical ability to act swiftly in an emergency.
