Heartland Alliance is Hiring!

 Community Health Worker

The Heartland Alliance is hiring a CHW to assist individuals who are experiencing homelessness, living with HIV/AIDs, have a serious mental illness, substance use disorder or are refugees with obtaining access to health care and social services in the community. The CHW will engage individuals who are not in care, are seeking care in emergency departments or other facilities, or are referred for care and met through outreach. The CHW will also provides on-going peer support and case management to obtain access to health care and achievement of collaboratively developed health goals proportionate to the individual’s readiness.


The Heartland Alliance is seeking a Case Manager to provide a broad range of case management services to populations including but not limited to children; adults; families; immigrants; refugees; and individuals with mental illness, HIV/AIDS and/or substance use issues. The Case Manager will be responsible for providing advocacy and assessment services to individuals, families and groups, conducting outreach and intake as assigned, and counseling individuals and families requiring assistance. The CHW will also secure information, such as medical, psychological, and social factors that could be contributing to the participant's situation, evaluate this information and then establish an appropriate service plan.
