CDPH Coordinator - Center on Halstead is hiring a Care Coordinator who will work under the supervision of the HIV Department Manager and provide intensive case management for participants including but not limited to conducting intakes, assessments, Individual Action Plans, and Linkage, Retention and Re-engagement to Care for people living with HIV and people vulnerable to HIV acquisition. The Care Coordinator will also provide appropriate active referrals and follow up and is responsible for fostering a safe and trauma informed environment.
Community Violence Clinical Advocate - The Center on Halstead is hiring a Clinical Advocate for LGBTQ survivors of Community Based and Domestic Violence providing LGBTQ centered direct services. Direct services include individual counseling, group counseling, crisis intervention, safety planning, case-management, advocacy, education, and outreach. This position is also responsible for cultivating relationships with Anti-Violence providers within the community and establishing community partnerships to facilitate client care.
